Background: Foundation models in healthcare are trained to predict observable outcomes based on historical data, which reflects existing clinical practices.

Question / Future Work: The implications of training models to imitate existing, potentially sub-optimal, clinical practices need to be fully understood. It is an open question whether training models to predict current documentation is the best approach for all healthcare deployment scenarios or if models should be adapted to suggest potentially superior or novel interventions.
